Instructions for Completing Health Assessments

For your convenience, SHIFT health assessments are being conducted at the Jubitz Truck Stop in Portland, Oregon (exit 307) (voted "World's Classiest Truck Stop" by the Travel Channel!). Your dispatcher will help get you to the testing site once your study prescreening is complete. You will need to wear certain types of clothes, and cholesterol and blood sugar tests require that you do not eat any food for 8 hrs prior to testing (water is OK and encouraged). Instructions for completing health assessments

Blood tests for cholesterol and glucose (sugar) will take place at Jubitz on the same day as your fitness tests and surveys. However, alternate back-up locations for blood tests have been identified along I-5 in case you have an unexpected time conflict or forget to fast for the test.
